Spring 2023 F-1 International Student Insurance Request Form

This is the Spring 2023 Elmhurst University-sponsored ISO insurance plan enrollment form for all F-1 international students admitted prior to Spring 2023.

• Spring 2023 insurance coverage dates are 01/01/2023-05/31/2023
• Spring 2023 insurance cost is $600

All F-1 international students who began their studies prior to Spring 2023 are strongly advised to enroll in the Elmhurst University-sponsored ISO health insurance plan. The high cost of health care in the U.S. puts international students at financial risk in the event of injury or sickness.

All F-1 international students admitted for Spring 2023 and beyond are required to purchase the Elmhurst University-sponsored ISO health insurance plan and will be automatically enrolled and billed each term through their Elmhurst University student accounts for the duration of their studies.

Students that complete this form will be enrolled and billed accordingly. An electronic copy of your insurance card will be emailed directly from ISO, as well as kept on file with Elmhurst University International Student Services.
